Atrial fibrillation (AF) and heart failure (HF) are global epidemics that began more than a century ago, and their association with an ageing general population has brought about an increase in cardiovascular morbidity and rising healthcare costs.1,2 More than 50 % of patients with permanent AF have a concurrent diagnosis of HF and this proportion is expected to rise.3 It is well established that…

In this short video interview, Prof Andreas Goette (St Vincenz Hospital Paderborn, Paderborn, DE) discusses the findings from the the EAST-AFNET 4 Trial. The trial investigated whether rhythm control therapy delivered within one year after AF diagnosis improves outcomes. The trial, presented at AHA 21, demonstrated that clinical benefit of early rhythm control was consistent in AF patterns.

Since the 1990s, the publication of several large randomised controlled trials (RCTs) have established the efficacy of implantable cardioverter defibrillator (ICD) therapy in reducing the risk of sudden cardiac death (SCD) in high-risk patients. Collectively these trials have demonstrated a significant all-cause mortality reduction compared with medical therapy alone, for both the primary and…

Catheter ablation therapy has been widely used for rhythm control in patients with atrial fibrillation(AF).1 Since the first report in 1994,2 several interventional techniques have been proposed for AF, including replication of the surgical Maze,2 targeting pulmonary vein (PV) foci,3 segmental ostial and circumferential PV isolation,4,5 ganglionated plexi ablation,6 linear lesions in the left…

Prevalence of AF and Obstructive Sleep Apnoea
Nonvalvular AF is the most common sustained arrhythmia, affecting nearly 3 million adult Americans.1–4 By 2050, nearly 12–15 million adults in the US will have AF.1 The global prevalence of obstructive sleep apnoea (OSA) is also increasing. OSA is the most common form of sleep-disordered breathing, affecting 10–15% of the general population (6–9% in…

The reported incidence of congenital heart disease (CHD) depends on the number of trivial lesions included, such as atrial and ventricular septal defects (ASDs and VSDs). Moderate-to-severe CHD numbers remain stable with 6 per 1,000 live births.1 Survival into adulthood has improved dramatically over the last 25 years and has been driven mainly by a decreased mortality in moderate and severe…

AF is the most common sustained cardiac arrhythmia and poses a significant public health challenge.1 In cases of AF, if sinus rhythm does not spontaneously return, cardioversion may be needed to alleviate symptoms and to improve cardiac performance.2 This may be performed by pharmacological methods, i.e. the administration of antiarrhythmic drugs, which is the preferred strategy in recent-onset…
